Shift Type: Day shift, Monday-Friday (exact start/end times vary; typical daytime hours). Split position between Prescott Village and Prescott Valley; frequent float between sites.

Roles & Responsibilities
- Evaluate, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders in the skilled nursing setting
- Develop individualized, evidence-based treatment plans that promote patient independence and quality of life
- Collaborate closely with nurses, therapists, physicians, and families to coordinate care and optimize outcomes
- Document skilled services clearly and timely in the medical record, meeting SNF regulatory and payer requirements
- Educate patients, families, and care teams on strategies to support communication and safe swallowing
- Participate in care planning and discharge planning to ensure continuity of care
Qualifications
- State SLP licensure in Arizona or eligibility to obtain Arizona licensure
- Master’s degree in Speech-Language Pathology or equivalent (required by many employers)
- ASHA CCC-SLP preferred but not required; new grads are welcome (years experience: any, including New Grad)
- Current BLS/CPR certification (or ability to obtain)
- Comfortable floating between two nearby facilities; reliable transportation recommended
- Strong clinical reasoning, documentation skills, and a team-oriented attitude
Education
- Master’s degree in Speech-Language Pathology (or equivalent graduate degree)
Knowledge & Skills
- Assessment and treatment of dysphagia and cognitive-communication disorders
- Experience with SNF/long-term care workflows, regulations, and documentation
- Proficiency with outcome-driven therapy planning and progress measurement
- Strong interdisciplinary communication and patient/family education skills
- Familiarity with electronic medical records and timely clinical documentation
