Roles & Responsibilities
- Assess patients' functional needs and develop individualized occupational therapy treatment plans
- Deliver hands-on therapeutic interventions that improve daily living skills and independence
- Document patient progress accurately and timely in the facility EHR
- Collaborate with nurses, PTs, SLPs, and the interdisciplinary team to coordinate care and discharge planning
- Educate patients and families on strategies, adaptive equipment, and home safety to promote long-term success
- Advocate for patient needs and modify interventions based on changing clinical status
- Participate in team meetings and contribute to quality improvement initiatives
Qualifications
- Current Minnesota Occupational Therapist license (required)
- NBCOT certification preferred or eligibility to obtain
- New grads welcome — any experience level considered
- BLS/CPR certification (or ability to obtain upon hire)
- Comfortable with floating between units and adapting to different patient populations
- Ability to pass background check and drug screening per facility requirements
Education
- Graduate of an accredited Occupational Therapy program (MOT/OTD or equivalent required for licensure)
Knowledge & Skills
- Skilled in adult and geriatric rehabilitation approaches
- Strong evaluation and treatment planning for SNF patients
- Proficient documentation practices and outcome tracking
- Familiarity with adaptive equipment, splinting basics, and ADL training
- Good communication and teamwork skills for interdisciplinary care
- Comfortable using electronic health records and common therapy software
