Roles & Responsibilities

• Assess residents’ functional abilities and create individualized occupational therapy plans
• Provide goal-driven interventions to improve ADLs, transfers, cognition, and independence
• Collaborate with nursing, rehab team members, and families to coordinate meaningful care
• Document progress clearly and timely in the facility’s records to support quality outcomes
• Educate residents and caregivers on adaptive techniques, equipment and safe home strategies
• Participate in interdisciplinary team meetings to advocate for patient-centered goals

Qualifications

• Licensed occupational therapist in New Jersey or eligible to obtain NJ licensure
• New grads welcome — entry-level clinicians encouraged to apply
• Experience in skilled nursing facility (SNF) settings is a plus but not required
• Strong documentation and communication skills to work effectively with interdisciplinary teams
• Current CPR/BLS certification or willingness to obtain per facility policy

Education

• Degree from an accredited Occupational Therapy program (MSOT/OTD or equivalent preferred)

Knowledge & Skills

• Skilled in functional assessments and developing individualized OT treatment plans
• Knowledge of ADL training, adaptive equipment, transfers and mobility techniques
• Familiarity with SNF rehab processes, restorative programs, and regulatory documentation
• Comfortable using electronic medical records for timely charting
• Excellent interpersonal skills for teaching residents and collaborating with staff and families
