Roles & Responsibilities
- Assess residents’ functional abilities and create individualized occupational therapy plans
- Provide goal-driven interventions to improve ADLs, transfers, cognition, and independence
- Collaborate with nursing, rehab team members, and families to coordinate meaningful care
- Document progress clearly and timely in the facility’s records to support quality outcomes
- Educate residents and caregivers on adaptive techniques, equipment and safe home strategies
- Participate in interdisciplinary team meetings to advocate for patient-centered goals
Qualifications
- Licensed occupational therapist in New Jersey or eligible to obtain NJ licensure
- New grads welcome — entry-level clinicians encouraged to apply
- Experience in skilled nursing facility (SNF) settings is a plus but not required
- Strong documentation and communication skills to work effectively with interdisciplinary teams
- Current CPR/BLS certification or willingness to obtain per facility policy
Education
- Degree from an accredited Occupational Therapy program (MSOT/OTD or equivalent preferred)
Knowledge & Skills
- Skilled in functional assessments and developing individualized OT treatment plans
- Knowledge of ADL training, adaptive equipment, transfers and mobility techniques
- Familiarity with SNF rehab processes, restorative programs, and regulatory documentation
- Comfortable using electronic medical records for timely charting
- Excellent interpersonal skills for teaching residents and collaborating with staff and families
