Roles & Responsibilities
- Provide excellent hands-on therapeutic care to residents in a skilled nursing facility setting
- Implement treatment plans developed by the supervising PT to help patients regain mobility and independence
- Monitor patient progress and document outcomes accurately and timely in the medical record
- Communicate clearly with interdisciplinary team members, residents, and families to coordinate care
- Perform safe transfers, gait training, and therapeutic exercise with an emphasis on resident safety and dignity
- Assist with discharge planning recommendations and functional goal-setting to optimize outcomes
Qualifications
- Licensed PTA in Oregon (or eligible to obtain state licensure prior to start)
- Graduated from an accredited Physical Therapist Assistant program
- CPR/BLS certification (or obtain upon hire) and up-to-date immunizations/background checks per facility policy
- Comfortable working in a SNF/long-term care setting; experience is welcome but new grads are encouraged to apply
- Strong documentation skills and ability to collaborate with an interdisciplinary team
Education
- Associate Degree from an accredited Physical Therapist Assistant program
- Completion of required clinical education and eligibility for state PTA licensure
Knowledge & Skills
- Knowledge of rehabilitation principles, transfers, gait training, and therapeutic exercise
- Familiarity with SNF workflows, Medicare/CMS documentation expectations, and progress reporting
- Good clinical judgment, patient safety focus, and infection control practices
- Strong verbal and written communication skills and EMR competency
- Ability to motivate residents and adapt plans to individual needs
