Roles & Responsibilities
- Provide skilled therapeutic interventions under the direction of a licensed physical therapist to improve patient mobility and function
- Assist with patient evaluations and implement portions of individualized treatment plans
- Document patient progress, treatment provided, and outcomes accurately and promptly
- Educate patients and families on home exercise programs and safety strategies to support recovery
- Collaborate with nurses, rehab staff, and interdisciplinary team members to coordinate care in the SNF setting
- Maintain therapy equipment and ensure a safe, clean treatment environment
- Advocate for patients and contribute to a positive, patient-centered culture
Qualifications
- Graduate of an accredited Physical Therapist Assistant program (Associate degree or equivalent)
- Valid PTA license in California or eligible to obtain state licensure
- BLS/CPR certification required or obtained upon hire
- Comfortable working in a Skilled Nursing Facility (SNF) environment
- Any experience level accepted, including new graduates — enthusiasm and willingness to learn welcomed
- Strong communication, documentation, and teamwork skills
Education
- Associate degree from an accredited Physical Therapist Assistant program or equivalent completion of an accredited PTA program
Knowledge & Skills
- Knowledge of therapeutic exercises, gait training, transfer techniques, and balance training
- Ability to follow and implement treatment plans developed by a PT
- Proficient clinical documentation and progress note writing
- Skilled in patient and family education with a compassionate bedside manner
- Familiarity with infection control, safety protocols, and SNF workflows
