Roles & Responsibilities

• Provide skilled therapeutic interventions under the direction of a licensed physical therapist to improve patient mobility and function
• Assist with patient evaluations and implement portions of individualized treatment plans
• Document patient progress, treatment provided, and outcomes accurately and promptly
• Educate patients and families on home exercise programs and safety strategies to support recovery
• Collaborate with nurses, rehab staff, and interdisciplinary team members to coordinate care in the SNF setting
• Maintain therapy equipment and ensure a safe, clean treatment environment
• Advocate for patients and contribute to a positive, patient-centered culture

Qualifications

• Graduate of an accredited Physical Therapist Assistant program (Associate degree or equivalent)
• Valid PTA license in California or eligible to obtain state licensure
• BLS/CPR certification required or obtained upon hire
• Comfortable working in a Skilled Nursing Facility (SNF) environment
• Any experience level accepted, including new graduates — enthusiasm and willingness to learn welcomed
• Strong communication, documentation, and teamwork skills

Education

• Associate degree from an accredited Physical Therapist Assistant program or equivalent completion of an accredited PTA program

Knowledge & Skills

• Knowledge of therapeutic exercises, gait training, transfer techniques, and balance training
• Ability to follow and implement treatment plans developed by a PT
• Proficient clinical documentation and progress note writing
• Skilled in patient and family education with a compassionate bedside manner
• Familiarity with infection control, safety protocols, and SNF workflows
