Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment to long-term care and SNF residents
• Create individualized treatment plans that improve mobility, function, and quality of life
• Collaborate closely with nursing and rehab teams to coordinate patient-centered care
• Document progress thoroughly in the NetHealth system and communicate outcomes with the care team
• Educate residents and families on safe mobility, fall prevention, and home exercise programs
• Maintain professional standards, safety protocols, and timely completion of documentation

Qualifications

• Active PT license in Texas (or eligible to obtain) — required to practice
• Doctor of Physical Therapy (DPT) or equivalent degree preferred
• 1-3 years of physical therapy experience, ideally in long-term care or SNF settings
• Comfortable using NetHealth or willing to learn the documentation system quickly
• Current BLS/CPR certification or willingness to obtain prior to start
• Strong communication and teamwork skills; ability to float between two facilities

Education

• DPT (Doctor of Physical Therapy) or equivalent from an accredited program preferred

Knowledge & Skills

• Skilled in geriatric and long-term care rehabilitation techniques
• Proficient in mobility assessment, transfers, gait training, and therapeutic exercise
• Experience with care planning and measurable goal setting
• Good clinical judgment with an emphasis on safety and fall prevention
• Able to use electronic documentation systems (NetHealth experience a plus)
• Comfortable working independently across multiple facility sites
