Roles & Responsibilities
- Provide skilled physical therapy evaluation and treatment to long-term care and SNF residents
- Create individualized treatment plans that improve mobility, function, and quality of life
- Collaborate closely with nursing and rehab teams to coordinate patient-centered care
- Document progress thoroughly in the NetHealth system and communicate outcomes with the care team
- Educate residents and families on safe mobility, fall prevention, and home exercise programs
- Maintain professional standards, safety protocols, and timely completion of documentation
Qualifications
- Active PT license in Texas (or eligible to obtain) — required to practice
- Doctor of Physical Therapy (DPT) or equivalent degree preferred
- 1-3 years of physical therapy experience, ideally in long-term care or SNF settings
- Comfortable using NetHealth or willing to learn the documentation system quickly
- Current BLS/CPR certification or willingness to obtain prior to start
- Strong communication and teamwork skills; ability to float between two facilities
Education
- DPT (Doctor of Physical Therapy) or equivalent from an accredited program preferred
Knowledge & Skills
- Skilled in geriatric and long-term care rehabilitation techniques
- Proficient in mobility assessment, transfers, gait training, and therapeutic exercise
- Experience with care planning and measurable goal setting
- Good clinical judgment with an emphasis on safety and fall prevention
- Able to use electronic documentation systems (NetHealth experience a plus)
- Comfortable working independently across multiple facility sites
