Roles & Responsibilities
- Provide skilled physical therapy services as a PTA for residents in a skilled nursing facility
- Collaborate with therapists, nursing staff, and interdisciplinary team to develop and carry out care plans
- Document patient progress accurately and in a timely manner following facility and CRG policies and procedures
- Meet productivity expectations and contribute to high-quality, efficient therapy delivery (therapy productivity requirement noted at 94%)
- Maintain professional presence in the facility and adhere to the dress code (black scrubs)
- Support resident-centered care and help improve functional outcomes for each patient
Qualifications
- Primary skill: PTA (Physical Therapist Assistant)
- 1-3 years of relevant experience (job lists 1-3 years)
- Experience working in a skilled nursing facility (SNF) or similar practice setting
- Ability to be on-site during scheduled hours to receive guaranteed hours
- Willingness to follow Continuum Rehabilitation Group / CRG policies and procedures
Knowledge & Skills
- Knowledge of skilled nursing facility therapy practices and resident care
- Strong documentation skills and ability to meet productivity targets
- Ability to collaborate effectively with interdisciplinary teams
- Professionalism in appearance and conduct (facility requires black scrubs and on-site presence)
- Familiarity with SNF workflows and regulatory expectations
