Roles & Responsibilities

• Provide skilled physical therapy services as a PTA for residents in a skilled nursing facility
• Collaborate with therapists, nursing staff, and interdisciplinary team to develop and carry out care plans
• Document patient progress accurately and in a timely manner following facility and CRG policies and procedures
• Meet productivity expectations and contribute to high-quality, efficient therapy delivery (therapy productivity requirement noted at 94%)
• Maintain professional presence in the facility and adhere to the dress code (black scrubs)
• Support resident-centered care and help improve functional outcomes for each patient

Qualifications

• Primary skill: PTA (Physical Therapist Assistant)
• 1-3 years of relevant experience (job lists 1-3 years)
• Experience working in a skilled nursing facility (SNF) or similar practice setting
• Ability to be on-site during scheduled hours to receive guaranteed hours
• Willingness to follow Continuum Rehabilitation Group / CRG policies and procedures

Knowledge & Skills

• Knowledge of skilled nursing facility therapy practices and resident care
• Strong documentation skills and ability to meet productivity targets
• Ability to collaborate effectively with interdisciplinary teams
• Professionalism in appearance and conduct (facility requires black scrubs and on-site presence)
• Familiarity with SNF workflows and regulatory expectations
