Roles & Responsibilities

• Provide skilled physical therapy interventions and carry out treatment plans to improve patient mobility and independence
• Perform patient assessments and document progress clearly and professionally
• Assist patients with therapeutic exercises, transfers, gait training, and functional mobility
• Collaborate with the interdisciplinary team to coordinate care and support patient-centered goals
• Educate patients and families on home programs, safety, and techniques to maximize outcomes
• Maintain timely, accurate documentation and comply with facility policies and regulatory standards
• Support discharge planning and transitions of care to promote safe, successful returns home

Qualifications

• PTA credential/qualification relevant to the role (PTA listed as core skill)
• Experience: any level welcome — including new grads
• Comfort working in a Skilled Nursing Facility (SNF) environment with frequent float assignments
• Strong communication, organization, and patient-handling skills
• Able to pass required background checks and facility onboarding

Knowledge & Skills

• Patient assessment and execution of treatment plans in a rehab/SNF setting
• Gait training, transfers, balance and strength interventions
• Safe patient handling techniques and use of adaptive equipment
• Accurate clinical documentation and time management
• Teamwork and clear communication with nursing and therapy colleagues
