Roles & Responsibilities
- Evaluate resident function and develop personalized PT treatment plans
- Implement evidence-based therapeutic interventions to improve mobility and independence
- Document patient progress clearly and timely in the facility EMR
- Collaborate with nursing and rehab team members to optimize resident outcomes
- Educate residents and families on home programs and safe mobility strategies
- Monitor and modify plans of care based on progress and clinical changes
- Promote a safe environment by following infection control and facility policies
Qualifications
- Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) — Massachusetts licensure or ability to obtain prior to start
- New grads welcome — any experience level considered, including recent graduates
- Current BLS/CPR certification (or obtain upon hire as required by facility)
- Comfort working in a Skilled Nursing Facility (SNF) setting; SNF experience a plus
- Strong communication skills and the ability to work well within an interdisciplinary team
Education
- Doctor of Physical Therapy (DPT) preferred; equivalent PT degree acceptable. New grads are welcome to apply.
Knowledge & Skills
- Clinical assessment and treatment planning for geriatric and post-acute populations
- Therapeutic exercise, gait training, transfers, and mobility assistance
- Skilled documentation practices and familiarity with facility EMR systems
- Knowledge of Medicare/SNF regulations and restorative care principles
- Ability to prioritize caseloads and adapt treatment to each resident's goals
