Roles & Responsibilities

• Perform thorough occupational therapy evaluations to identify patient needs and goals
• Develop and implement individualized treatment plans that promote independence
• Deliver daily treatments, documenting progress and adjusting plans as needed
• Manage safe and timely discharges with clear home program recommendations
• Collaborate with the interdisciplinary rehab team to optimize patient outcomes
• Meet productivity and documentation standards while maintaining high-quality care

Qualifications

• Active Occupational Therapy license in the state of practice or eligible for licensure
• 1-3 years of OT experience preferred; prior SNF experience strongly preferred
• Comfortable performing evaluations, daily treatments, and discharges in a SNF setting
• Ability to follow facility policies and meet productivity requirement (90%)
• Willingness to work onsite and follow dress code (black scrubs as specified)
• Strong communication and teamwork skills

Education

• Occupational Therapy degree (required to meet licensure requirements)

Knowledge & Skills

• Skilled in OT evaluations, treatment planning, and discharge planning
• Experience working in Skilled Nursing Facility (SNF) environments preferred
• Familiarity with rehab documentation and productivity expectations
• Ability to prioritize caseloads and manage time effectively
• Comfortable working as part of an interdisciplinary team
