Roles & Responsibilities
- Evaluate patients’ communication and swallowing needs using evidence-based assessments
- Develop and implement individualized treatment plans that drive measurable progress
- Provide skilled dysphagia management and swallowing therapy when indicated
- Collaborate with nursing, rehab staff, and families to coordinate person-centered care
- Document timely, clear progress notes and discharge summaries in the facility’s system
- Educate patients, families, and staff to support carryover and safe swallowing strategies
- Participate in interdisciplinary meetings to advocate for patient goals and outcomes
Qualifications
- Certificate of Clinical Competence (CCC) from ASHA — listed as a primary requirement
- 1-3 years of SLP experience (experience in skilled nursing facility settings preferred)
- Current state licensure in Speech-Language Pathology (or ability to obtain NH license quickly)
- Active professional liability insurance or ability to obtain coverage
- Strong clinical reasoning, excellent documentation skills, and team-oriented mindset
Education
- Master’s degree in Speech-Language Pathology preferred (or equivalent clinical training)
- ASHA CCC required as noted in the job details
Knowledge & Skills
- Skilled in adult dysphagia evaluation and treatment
- Experience with cognitive-communication disorders and therapy strategies
- Familiarity with SNF workflows, Medicare documentation, and interdisciplinary care
- Comfortable with electronic health record documentation and timely charting
- Strong interpersonal skills for patient/family education and team collaboration
