Roles & Responsibilities
- Provide skilled physical therapy evaluation and develop individualized treatment plans
- Deliver hands-on and therapeutic interventions to help residents regain function and independence
- Document timely, clear, and accurate treatment notes and progress toward goals
- Collaborate with nursing and interdisciplinary teams to coordinate patient-centered care
- Educate patients and caregivers on safe mobility, home programs, and fall prevention
- Monitor outcomes and modify treatment plans based on patient response and facility goals
Qualifications
- Valid PT license in New Hampshire or ability to obtain state licensure promptly
- DPT degree or equivalent credential (new graduates are welcome to apply)
- Comfort working in a Skilled Nursing Facility (SNF) setting; previous SNF experience is a plus
- Current BLS/CPR certification or willingness to obtain prior to start
- Strong clinical assessment, treatment planning, and documentation skills
- Reliable, professional, and collaborative approach to resident care
Education
- Doctor of Physical Therapy (DPT) preferred; PT license required or eligible
Knowledge & Skills
- Skilled in mobility training, therapeutic exercise, transfers, and gait training
- Proficient in developing and progressing home exercise programs and discharge plans
- Knowledgeable in Medicare/rehab documentation and SNF regulatory expectations
- Strong communication skills with residents, families, and interdisciplinary teams
- Ability to prioritize caseloads and manage time efficiently in a busy SNF
