Roles & Responsibilities

• Provide high-quality skilled physical therapy services to residents in a skilled nursing facility
• Implement and carry out treatment plans developed by the therapy team, focusing on patient-centered goals
• Document patient progress accurately and timely in accordance with facility and CRG policies
• Collaborate with nurses, CNAs, and interdisciplinary team members to ensure coordinated care
• Maintain productivity expectations (therapist productivity requirement: 92%) while delivering compassionate care
• Adhere to facility dress code (black scrubs) and be present on-site to receive guaranteed hours
• Follow Continuum Rehabilitation Group policies and procedures to ensure compliance and quality

Qualifications

• PTA (Physical Therapist Assistant) – position is for a PTA
• 1–3 years of relevant experience preferred (as listed: Years Experience: 1-3)
• Experience or comfort working in a skilled nursing facility (SNF) setting
• Ability to be on-site for scheduled hours and follow facility requirements
• Willingness to meet productivity expectations and follow CRG P&P
• Professional dress in black scrubs as required by the facility

Knowledge & Skills

• Skilled in physical therapy techniques appropriate for SNF residents
• Documentation and charting skills consistent with facility standards
• Strong communication and teamwork with interdisciplinary staff
• Ability to manage caseloads while meeting productivity targets
• Familiarity with SNF workflows and regulatory expectations
