Roles & Responsibilities
- Provide high-quality skilled physical therapy services to residents in a skilled nursing facility
- Implement and carry out treatment plans developed by the therapy team, focusing on patient-centered goals
- Document patient progress accurately and timely in accordance with facility and CRG policies
- Collaborate with nurses, CNAs, and interdisciplinary team members to ensure coordinated care
- Maintain productivity expectations (therapist productivity requirement: 92%) while delivering compassionate care
- Adhere to facility dress code (black scrubs) and be present on-site to receive guaranteed hours
- Follow Continuum Rehabilitation Group policies and procedures to ensure compliance and quality
Qualifications
- PTA (Physical Therapist Assistant) – position is for a PTA
- 1–3 years of relevant experience preferred (as listed: Years Experience: 1-3)
- Experience or comfort working in a skilled nursing facility (SNF) setting
- Ability to be on-site for scheduled hours and follow facility requirements
- Willingness to meet productivity expectations and follow CRG P&P
- Professional dress in black scrubs as required by the facility
Knowledge & Skills
- Skilled in physical therapy techniques appropriate for SNF residents
- Documentation and charting skills consistent with facility standards
- Strong communication and teamwork with interdisciplinary staff
- Ability to manage caseloads while meeting productivity targets
- Familiarity with SNF workflows and regulatory expectations
