Roles & Responsibilities
- Assess patients' mobility, strength, balance and functional needs to build personalized plans of care
- Develop and implement evidence-based treatment plans to improve patient outcomes
- Provide hands-on therapeutic interventions including therapeutic exercise and manual therapy
- Monitor progress and adjust care plans with measurable goals and clear documentation
- Educate patients and families on injury prevention, home exercise programs and safe discharge plans
- Collaborate with interdisciplinary team members to coordinate comprehensive care
- Maintain timely, accurate documentation in the facility's EHR and ensure compliance with regulatory standards
Qualifications
- Active California Physical Therapy license required (or ability to obtain prior to start)
- Graduation from an accredited Physical Therapy program; DPT preferred
- BLS/CPR certification required or willingness to obtain
- Comfortable with electronic health records and timely documentation
- Strong clinical reasoning, communication, and teamwork skills
- Previous outpatient or acute-care PT experience preferred
Education
- Doctor of Physical Therapy (DPT) or degree from an accredited Physical Therapy program
Knowledge & Skills
- Skilled in therapeutic exercise, manual therapy, gait training and functional mobility
- Knowledge of musculoskeletal and neuromuscular evaluation and treatment
- Proficient in patient education and home exercise program design
- Familiarity with documentation standards, ICD-10/CPT basics and regulatory compliance
- Excellent time management, prioritization and interdisciplinary communication
