Roles & Responsibilities

• Evaluate and treat outpatient physical therapy patients with professional, evidence-based care
• Develop individualized treatment plans and document progress clearly and promptly
• Provide direct patient care across outpatient and occasional inpatient settings including ER and PACU discharges
• Collaborate with interdisciplinary teams to coordinate continuity of care
• Educate patients and caregivers to empower recovery and self-management
• Participate in weekend and holiday rotations as scheduled to support inpatient needs
• Maintain professional standards, safety protocols, and accurate billing/documentation
• Support satellite clinic operations when assigned, bringing flexibility and positive patient experience

Qualifications

• Licensed Physical Therapist in Vermont or eligible for Vermont licensure
• Current CPR/BLS certification (or willingness to obtain upon hire)
• Experience in outpatient physical therapy; experience with inpatient, ER, or PACU care is a strong plus
• Comfortable treating a broad adult caseload; pelvic floor experience is highly desirable but not required
• Strong documentation skills and experience with electronic medical records
• Excellent communication and teamwork skills; ability to adapt to multiple clinic sites

Education

• Doctor of Physical Therapy (DPT) preferred; Master or Bachelor of Physical Therapy considered

Knowledge & Skills

• Patient evaluation and assessment
• Treatment planning and direct patient care
• Outpatient rehabilitation techniques and modalities
• Familiarity with inpatient transitions, ER care, and PACU discharges
• Pelvic floor therapy knowledge (preferred)
• Ability to work across satellite clinics and varied workflows
• Professional documentation and EMR usage
• Strong patient education and interdisciplinary collaboration skills
