Roles & Responsibilities
- Assess patients comprehensively and develop individualized, compassionate hospice care plans
- Coordinate transitions of care between hospital, home, rehab, and long-term care settings
- Advocate for patients and families to secure necessary services and community resources
- Collaborate with physicians, therapists, social workers, and payers to ensure continuity of care
- Evaluate medical necessity and work with insurers to obtain authorizations when needed
- Document assessments, interventions, and outcomes thoroughly and accurately
- Monitor length of stay and identify barriers to safe discharge or ongoing care
- Facilitate patient and family education about diagnoses, treatments, and self-care
Qualifications
- Active Oregon RN license (required)
- Graduate of an accredited school of nursing, social work, medicine, or related healthcare program (required)
- BSN or MSN preferred
- Case Management Certification preferred
- Experience in hospice nursing, case management, care coordination, or discharge planning preferred
- Strong communication skills and the ability to work effectively with diverse populations
Education
- Graduate of an accredited school of nursing, social work, medicine, or related medical occupation
- BSN/MSN preferred
Knowledge & Skills
- Patient assessment and comprehensive care planning
- Care coordination and discharge planning across care settings
- Clinical documentation and tracking of outcomes/quality metrics
- Interdisciplinary collaboration and team-based care
- Advocacy for patients and families and knowledge of community resources
- Familiarity with insurance authorization and utilization management processes
- Ability to educate patients and families with empathy and clarity
