Roles & Responsibilities

• Evaluate residents and develop individualized therapeutic plans of care with clear short- and long-term goals
• Deliver evidence-based therapeutic interventions and monitor progress toward functional independence
• Document all evaluative and therapeutic interactions in the Electronic Medical Record in a clear, timely manner
• Recommend and fit appropriate assistive devices and ensure residents and caregivers are trained in their use
• Consult with physicians and the interdisciplinary team on plan of care and discharge needs; participate in team meetings
• Supervise and mentor assistant-level staff and new hires during orientation to maintain quality care
• Provide in-service education and help develop new rehabilitation programs and services
• Ensure compliance with Medicare, Medicaid, and other reimbursement requirements and infection control procedures

Qualifications

• Active PT license in Massachusetts required
• Bachelor's degree required; Master’s degree or Doctorate (DPT) preferred
• Basic Life Support (BLS) certification preferred or willing to obtain
• Prior experience in long-term care or skilled nursing facilities preferred
• Proven ability to document in EMR and knowledge of Medicare/Medicaid documentation requirements
• Comfort supervising assistants and collaborating within an interdisciplinary team
• Ability to perform physical demands of the job, including lifting up to 50 pounds

Education

• Bachelor's degree required
• Master's degree or Doctorate (DPT) preferred

Knowledge & Skills

• Patient assessment and development of therapeutic plans of care
• Therapeutic modalities and assistive device recommendations
• Strong EMR documentation skills and Microsoft Office proficiency
• Familiarity with Medicare, Medicaid, and reimbursement regulations
• Excellent verbal, written, and presentation communication skills
• Time management, clinical decision-making, and resident-centered care approach
• Infection control practices and ability to provide in-service education
