Roles & Responsibilities

• Provide high-quality occupational therapy evaluations and individualized treatment plans for older adult residents
• Deliver therapeutic interventions that improve mobility, safety, and residents' overall quality of life
• Collaborate closely with rehab team members, nursing staff, and interdisciplinary partners to coordinate care
• Document evaluations, treatments, and progress notes accurately using Net Health
• Support and promote a resident-centered culture that prioritizes dignity and independence
• Educate residents and families on adaptive strategies, home safety, and recommended equipment
• Participate in team meetings and contribute to care planning and discharge recommendations
• Maintain professional standards and comply with facility policies, infection control, and safety protocols

Qualifications

• Current PA Occupational Therapist license in good standing
• CPR certification required
• Two-step PPD or TB skin test within 30 days of start date
• Covid-19 vaccination record or documented exemption
• Successful background check and fingerprinting
• Experience working in senior care or skilled nursing preferred
• Strong communication and teamwork skills with a compassionate, resident-focused approach
• Comfort with electronic documentation; Net Health training available if needed
• Resume and professional references

Knowledge & Skills

• Skilled in occupational therapy evaluations and outcome-driven treatment planning
• Proficient documentation practices and clinical charting
• Experience with resident-centered care and geriatric populations
• Ability to collaborate effectively with rehab, nursing, and interdisciplinary teams
• Knowledge of ADL training, adaptive equipment, and fall prevention strategies
• Strong clinical reasoning, time management, and communication skills
• Familiarity with infection control, safety protocols, and regulatory requirements
