Roles & Responsibilities

• Evaluate residents and establish individualized therapeutic plans of care
• Develop functional long- and short-term goals including frequency and duration of services
• Deliver evidence-based therapeutic interventions to maximize independence in activities of daily living
• Document all evaluative and therapeutic interactions clearly in the Electronic Medical Record
• Recommend and instruct residents on appropriate assistive devices
• Collaborate with physicians, nursing staff, and the interdisciplinary team regarding plans of care and discharge needs
• Monitor treatment effectiveness and modify plans to meet resident goals
• Supervise Physical Therapist Assistant and assistant-level staff as appropriate
• Provide in-service education and mentorship to new staff and contribute to program development
• Ensure compliance with Medicare, Medicaid, and other reimbursement regulations

Qualifications

• Active license as a Physical Therapist in Massachusetts
• Bachelor's degree required; Master's or Doctorate preferred
• Basic Life Support (BLS) certification preferred
• Prior experience in long-term care or skilled nursing preferred
• Demonstrated proficiency with EMR documentation and Medicare/Medicaid documentation requirements
• Ability to lift up to 50 pounds and perform the physical demands of the role
• Strong verbal and written English communication skills

Education

• Bachelor's degree required
• Master's or Doctorate in Physical Therapy preferred
• An equivalent combination of education and experience may be considered

Knowledge & Skills

• Skilled in patient evaluation and therapeutic plan of care development
• Proficient in EMR documentation and Microsoft Office
• Knowledgeable about Medicare, Medicaid, and reimbursement regulations
• Experience recommending and training on assistive devices
• Excellent interpersonal, presentation, and time-management skills
• Comfortable working collaboratively within an interdisciplinary team
• Ability to supervise assistants and provide staff mentorship
