Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Provide high-quality physical therapy evaluations and individualized treatment plans that enhance mobility and safety
- Deliver compassionate care across skilled nursing, personal care, and independent living settings
- Collaborate closely with nursing, rehab team members, and other healthcare partners to optimize resident outcomes
- Document clinical services accurately and timely using Net Health (training available)
- Support a resident-centered culture by coaching residents and families on safe mobility and independence
- Participate in interdisciplinary meetings to coordinate care and improve quality of life
Qualifications
- PA Physical Therapist license in good standing
- CPR certification through the American Heart Association or the Red Cross
- Experience working in senior care or geriatrics preferred
- Two-step PPD or TB skin test within 30 days of start date
- Covid vaccination records or documented exemption
- Background check and fingerprinting clearance
- Current resume and a physical performed by a physician, PA, or nurse practitioner
Knowledge & Skills
- Strong clinical documentation skills and comfort using electronic health records (Net Health)
- Expertise in physical therapy evaluations, treatment planning, and interventions for older adults
- Excellent communication and teamwork to collaborate with interdisciplinary staff
- Compassionate, patient-focused approach with strong resident advocacy
- Ability to prioritize caseloads and adapt care across multiple levels of senior living
