Roles & Responsibilities
- Provide therapeutic procedures under the supervising therapist to help residents reach functional goals
- Implement and progress treatment plans while monitoring outcomes and communicating changes
- Educate residents and caregivers on exercises, assistive devices, and safe mobility techniques
- Document all therapeutic interactions accurately and in a timely manner to support continuity of care
- Collaborate with the interdisciplinary team and attend relevant meetings to coordinate resident care
- Recommend and fit assistive devices and ensure residents understand their safe use
- Follow infection control and department protocols to maintain a clean, safe environment
- Contribute to program development and continuous improvement in rehabilitation services
Qualifications
- Associate degree (required)
- Active PT Assistant license (required)
- Basic Life Support (BLS) certification (required)
- Prior experience in long-term care or skilled nursing facility preferred
- Ability to lift up to 50 pounds and perform usual physical demands of the role
- Strong verbal and written English communication skills
Education
- Associate degree required (Physical Therapy Assistant or related field)
Knowledge & Skills
- Therapeutic procedures and treatment plan execution
- Patient education and assistive device recommendation and training
- Accurate recordkeeping and clinical documentation
- Infection control procedures and safe patient handling
- Proficient with Microsoft Office and basic computer documentation
- Interdisciplinary team collaboration, time management, and customer service
- Strong interpersonal, presentation, and analytical decision-making skills
