Roles & Responsibilities
- Evaluate residents and create individualized treatment plans that maximize functional independence
- Deliver therapeutic interventions and use appropriate modalities to meet established goals
- Document all evaluative and therapeutic interactions in the EMR clearly and timely
- Recommend and fit assistive devices, and educate residents and caregivers on their use
- Collaborate with physicians, nurses, and the interdisciplinary team on care planning and discharge needs
- Supervise and mentor PTA and assistant-level staff, ensuring safe, compliant practice
- Provide in-service education and help develop new rehabilitation programs and services
- Monitor treatment outcomes and modify plans to ensure progress toward functional goals
Qualifications
- Active license as a Physical Therapist in the State of Massachusetts (required)
- Bachelor’s degree required; Master’s or Doctorate preferred
- BLS certification preferred (or ability to obtain)
- Prior experience in long-term care or skilled nursing preferred
- Experience supervising PT Assistants and working within interdisciplinary teams
- Strong documentation skills and working knowledge of Medicare/Medicaid reimbursement
Education
- Bachelor's degree required
- Master's or Doctorate in Physical Therapy preferred
Knowledge & Skills
- Patient evaluation and treatment planning expertise
- Proficient EMR documentation and excellent time management
- Assistive device recommendation and training
- Understanding of Medicare, Medicaid, and reimbursement regulations
- Strong verbal, written, and presentation communication skills
- Proficient in Microsoft Office and comfortable with clinical software
- Compassionate, resident-centered approach and strong interpersonal skills
