Roles & Responsibilities

• Evaluate patients across inpatient, swing bed, outpatient and skilled nursing settings using sound clinical judgment
• Develop and implement personalized treatment plans to improve patient function and independence
• Perform thorough functional and cognitive assessments and document findings in the EMR
• Manage a diverse caseload independently while coordinating care with the multidisciplinary team
• Educate patients and families on therapeutic goals, home programs and safe independence strategies
• Monitor progress, adjust treatment plans as needed, and communicate outcomes to the care team

Qualifications

• Active California Occupational Therapist License in good standing
• Current AHA Healthcare Provider BLS certification
• Bachelor’s, Master’s, or Doctorate in Occupational Therapy from an accredited program
• Preferred 2-3 years experience in acute care, rehabilitation, or outpatient settings
• Strong communication and interpersonal skills with proven caseload management
• Proficiency with EMR documentation and basic computer skills

Education

• Bachelor’s, Master’s, or Doctorate in Occupational Therapy (required)

Knowledge & Skills

• Clinical assessment and treatment planning across varied care settings
• Patient-centered care and compassionate therapeutic approaches
• EMR documentation best practices and accurate clinical charting
• Analytical thinking and problem-solving for complex cases
• Ability to collaborate effectively with interdisciplinary teams
