Roles & Responsibilities

• Assess patients using evidence-based evaluation techniques and set clear functional goals
• Develop and implement individualized treatment plans that improve mobility and independence
• Document patient encounters thoroughly and accurately to support continuity of care
• Delegate and supervise supportive staff, volunteers, and students to ensure safe, high-quality care
• Coordinate care with interdisciplinary team members to optimize patient outcomes
• Advocate for patients by ensuring their needs and preferences are represented in care planning
• Float to other departments as needed, providing competent patient care across settings

Qualifications

• Current or eligible Physical Therapist license for Oregon (facility will assist with credentialing as needed)
• Graduation from an accredited physical therapy program (DPT preferred)
• Basic Life Support (BLS) certification, or willingness to obtain prior to start
• Proven skills in outpatient evaluation, treatment planning, and documentation
• Comfort supervising and mentoring staff, volunteers, and students
• Ability to adapt and float to other departments when qualified

Education

• Graduation from an accredited physical therapy program (Doctor of Physical Therapy preferred)

• Patient assessment and outcome measurement
• Treatment planning and therapeutic interventions
• Accurate patient documentation and charting
• Delegation and team supervision
• Care coordination and interdisciplinary communication
• Outpatient care workflows and priorities
• Experience working with volunteers and students
