Roles & Responsibilities
- Provide compassionate, skilled occupational therapy services to short-term rehabilitation residents
- Assess patient function and create individualized treatment plans that promote independence
- Implement therapy interventions and document patient progress clearly and timely
- Collaborate with nurses, PT, SLP, and care team members to coordinate outstanding patient care
- Educate patients and families on safe strategies and home modifications to support discharge goals
- Advocate for patient needs while maintaining regulatory and safety compliance
Qualifications
- Licensed or certified COTA in good standing; ability to obtain Minnesota credentials if not already licensed
- Graduation from an accredited Occupational Therapy Assistant program
- Basic Life Support (BLS/CPR) certification or willingness to obtain
- Comfortable working in a skilled nursing facility setting; new graduates are welcome
- Strong documentation skills and familiarity with clinical charting expectations
- Excellent communication and teamwork skills
Education
- Associate degree from an accredited Occupational Therapy Assistant program (OTA)
Knowledge & Skills
- Skilled in ADL training, splinting basics, therapeutic activities, and adaptive equipment
- Knowledge of geriatric rehabilitation and SNF care processes
- Ability to evaluate functional mobility and recommend safe discharge plans
- Competent in documentation and outcome-driven goal writing
- Team-oriented approach with strong clinical reasoning and time management
