Roles & Responsibilities

• Provide skilled physical therapy interventions under PT supervision to help patients regain mobility and independence
• Assist with therapeutic exercises, gait training, transfers and ADL retraining for SNF residents
• Monitor patient progress and report changes to the supervising Physical Therapist
• Document treatments, progress notes and discharge planning accurately in the patient record
• Collaborate with nursing and interdisciplinary team members to support resident care goals
• Educate patients and families on home exercise programs and safe mobility techniques

Qualifications

• Current or eligible Physical Therapist Assistant license for Ohio (state licensure required prior to start)
• BLS/CPR certification (or willingness to obtain)
• New grads welcome — any level of experience considered
• Ability to pass background check, TB screening and required immunizations
• Comfort working in a skilled nursing facility (SNF) setting and partnering with an interdisciplinary team

Education

• Associate degree from an accredited Physical Therapist Assistant program and successful completion of required licensure

Knowledge & Skills

• Skilled in therapeutic exercise, gait training and balance interventions
• Strong clinical documentation and time-management abilities
• Good communication skills with residents, families and clinical team members
• Familiarity with safety and infection-control practices in long-term care
• Ability to follow care plans and adapt interventions to individual resident needs
