Roles & Responsibilities

• Provide skilled occupational therapy evaluations and develop individualized treatment plans
• Deliver evidence-based therapeutic interventions to promote independence in daily activities
• Collaborate with interdisciplinary team members to coordinate patient-centered care
• Document care and progress clearly and timely in the facility’s medical record
• Educate patients, families, and caregivers on techniques to maximize functional outcomes
• Assist with discharge planning and recommendations to support safe transitions

Qualifications

• Current and unrestricted Occupational Therapy license in Ohio (or compact/state licensure as applicable)
• Registered/Certified OT credentials (OTR/L) per state requirements
• New graduates welcome as the posting accepts any experience level
• Basic Life Support (BLS) certification preferred or willing to obtain
• Ability to pass required background checks, drug screen, and facility onboarding
• Strong communication and documentation skills

Education

• Occupational Therapy degree required as needed for state licensure (per Ohio requirements)

Knowledge & Skills

• Skilled in OT evaluation techniques and developing measurable goals
• Experience with therapeutic exercises, ADL retraining, and adaptive equipment
• Comfortable working in a skilled nursing facility setting and collaborating with nursing staff
• Good clinical reasoning, time management, and prioritization skills
• Professional bedside manner and the ability to teach and motivate patients
