Roles & Responsibilities

• Provide skilled physical therapy interventions under the supervision of the treating therapist to improve patient mobility and function
• Deliver therapeutic exercise, gait training, transfers, and assistive device training with compassion and clinical judgment
• Document patient progress, treatment plans, and skilled interventions accurately and on time
• Collaborate with nurses, CNAs, and other therapists to coordinate safe, effective care plans
• Educate patients and families on home exercise programs and strategies to maximize independence
• Participate in team meetings and communicate changes in patient status promptly
• Support discharge planning by recommending safe mobility and equipment needs

Qualifications

• Graduate of an accredited Physical Therapist Assistant program or equivalent
• Licensed or eligible for PTA practice in New Jersey (facility will confirm specific state licensure requirements)
• CPR/BLS certification required or obtained upon hire
• Open to clinicians at all experience levels — new grads are welcome
• Strong interpersonal skills and ability to work in a skilled nursing facility environment
• Ability to pass facility background checks, drug screens, and required onboarding

Education

• Completion of an accredited Physical Therapist Assistant program (Associate degree or equivalent)

Knowledge & Skills

• Skilled in therapeutic exercise, transfer training, gait training, and use of assistive devices
• Knowledge of documentation best practices and rehabilitation outcome tracking
• Comfort working with geriatric and complex medical populations in a SNF setting
• Ability to follow care plans, take direction from supervising therapists, and adapt treatment to patient needs
• Good clinical reasoning, time management, and teamwork skills
