Roles & Responsibilities
- Provide skilled physical therapy interventions under the supervision of the treating therapist to improve patient mobility and function
- Deliver therapeutic exercise, gait training, transfers, and assistive device training with compassion and clinical judgment
- Document patient progress, treatment plans, and skilled interventions accurately and on time
- Collaborate with nurses, CNAs, and other therapists to coordinate safe, effective care plans
- Educate patients and families on home exercise programs and strategies to maximize independence
- Participate in team meetings and communicate changes in patient status promptly
- Support discharge planning by recommending safe mobility and equipment needs
Qualifications
- Graduate of an accredited Physical Therapist Assistant program or equivalent
- Licensed or eligible for PTA practice in New Jersey (facility will confirm specific state licensure requirements)
- CPR/BLS certification required or obtained upon hire
- Open to clinicians at all experience levels — new grads are welcome
- Strong interpersonal skills and ability to work in a skilled nursing facility environment
- Ability to pass facility background checks, drug screens, and required onboarding
Education
- Completion of an accredited Physical Therapist Assistant program (Associate degree or equivalent)
Knowledge & Skills
- Skilled in therapeutic exercise, transfer training, gait training, and use of assistive devices
- Knowledge of documentation best practices and rehabilitation outcome tracking
- Comfort working with geriatric and complex medical populations in a SNF setting
- Ability to follow care plans, take direction from supervising therapists, and adapt treatment to patient needs
- Good clinical reasoning, time management, and teamwork skills
