Roles & Responsibilities
- Conduct thorough speech-language evaluations to identify patient needs and goals
- Provide daily individualized treatments to help patients regain communication and swallowing function
- Manage and complete discharges with clear aftercare plans and caregiver education
- Document timely, accurate clinical notes and maintain productivity expectations
- Collaborate with an interdisciplinary therapy team to optimize patient outcomes
- Follow Continuum Rehabilitation Group policies and facility procedures, including dress code
Qualifications
- Licensed or license-eligible Speech-Language Pathologist (SLP) for practice in New Mexico
- 1-3 years of professional experience; prior SNF experience strongly preferred
- Comfortable working in a skilled nursing facility environment and providing evaluations, daily treatments, and discharges
- Willingness to follow company policies and meet productivity requirements (90%)
- Ability to be on-site and maintain professional appearance (facility dress code: black scrubs)
Knowledge & Skills
- Skilled in assessment and treatment planning for adult speech, language, cognition, and swallowing disorders
- Strong documentation and time management skills to meet productivity targets
- Collaborative communication with nurses, therapists, families, and facility staff
- Ability to adapt treatment approaches for SNF patients and varying caseloads
- Professionalism and reliability to maintain on-site coverage and guaranteed hours
