Candidate Pay Rate: $63 per hour

Roles & Responsibilities
- Evaluate patients to identify physical impairments and functional limitations and develop individualized treatment plans
- Provide hands-on and exercise-based interventions to improve mobility, balance and independence
- Use evidence-based orthopedic and neurological approaches to address chronic pain and movement deficits
- Collaborate with the interdisciplinary rehab team to coordinate care and progress toward return-to-function goals
- Document assessments, interventions and patient progress clearly and timely
- Educate patients and caregivers on home programs, safety strategies and community resources to support long-term success
Qualifications
- Current unencumbered Oregon Physical Therapy license required
- Healthcare Provider Level BLS required
- Comfortable traveling to multiple points of care as part of the assignment
- Experience in orthopedic and neurological/ stroke rehab, balance/dizziness rehab, or chronic pain treatment is a strong plus
- Strong communication skills and a patient-centered approach
Education
- Graduation from an accredited Physical Therapy program (DPT preferred); must meet Oregon licensing requirements
Knowledge & Skills
- Skilled in functional mobility assessment and therapeutic exercise prescription
- Familiarity with balance and vestibular rehabilitation techniques
- Ability to design and progress individualized home exercise programs
- Knowledge of safety and fall prevention strategies for community return
- Effective interdisciplinary collaboration and patient education skills
