Roles & Responsibilities

• Evaluate patients to identify physical impairments and functional limitations and develop individualized treatment plans
• Provide hands-on and exercise-based interventions to improve mobility, balance and independence
• Use evidence-based orthopedic and neurological approaches to address chronic pain and movement deficits
• Collaborate with the interdisciplinary rehab team to coordinate care and progress toward return-to-function goals
• Document assessments, interventions and patient progress clearly and timely
• Educate patients and caregivers on home programs, safety strategies and community resources to support long-term success

Qualifications

• Current unencumbered Oregon Physical Therapy license required
• Healthcare Provider Level BLS required
• Comfortable traveling to multiple points of care as part of the assignment
• Experience in orthopedic and neurological/ stroke rehab, balance/dizziness rehab, or chronic pain treatment is a strong plus
• Strong communication skills and a patient-centered approach

Education

• Graduation from an accredited Physical Therapy program (DPT preferred); must meet Oregon licensing requirements

Knowledge & Skills

• Skilled in functional mobility assessment and therapeutic exercise prescription
• Familiarity with balance and vestibular rehabilitation techniques
• Ability to design and progress individualized home exercise programs
• Knowledge of safety and fall prevention strategies for community return
• Effective interdisciplinary collaboration and patient education skills
