Roles & Responsibilities
- Perform comprehensive home health assessments and OASIS documentation with accuracy and compassion
- Develop, implement, and coordinate individualized care plans working closely with patients and families
- Manage caseload efficiently, prioritizing patient safety and timely interventions
- Communicate effectively with interdisciplinary teams, physicians, and community resources to ensure continuity of care
- Educate patients and caregivers on disease management, medications, and self-care to improve outcomes
- Monitor clinical progress, identify changes in condition, and escalate concerns promptly
Qualifications
- Active Registered Nurse (RN) license eligible to practice in Vermont
- OASIS experience required
- Home health and case management experience required
- Comfortable working as a travel/contract nurse; experienced travelers strongly preferred
- Ability to commit to the full contract period with no requested time off
- Current BLS certification (or willingness to obtain prior to start)
Education
- Nursing degree (ADN or BSN) and active RN license
Knowledge & Skills
- Proven experience with OASIS assessments and home health documentation
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Care coordination skills and familiarity with community resources
- Comfortable with electronic medical records and timely documentation
- Ability to manage a caseload independently while maintaining high-quality care
