Roles & Responsibilities

• Perform comprehensive home health assessments and OASIS documentation with accuracy and compassion
• Develop, implement, and coordinate individualized care plans working closely with patients and families
• Manage caseload efficiently, prioritizing patient safety and timely interventions
• Communicate effectively with interdisciplinary teams, physicians, and community resources to ensure continuity of care
• Educate patients and caregivers on disease management, medications, and self-care to improve outcomes
• Monitor clinical progress, identify changes in condition, and escalate concerns promptly

Qualifications

• Active Registered Nurse (RN) license eligible to practice in Vermont
• OASIS experience required
• Home health and case management experience required
• Comfortable working as a travel/contract nurse; experienced travelers strongly preferred
• Ability to commit to the full contract period with no requested time off
• Current BLS certification (or willingness to obtain prior to start)

Education

• Nursing degree (ADN or BSN) and active RN license

Knowledge & Skills

• Proven experience with OASIS assessments and home health documentation
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Care coordination skills and familiarity with community resources
• Comfortable with electronic medical records and timely documentation
• Ability to manage a caseload independently while maintaining high-quality care
