Roles & Responsibilities

• Perform comprehensive start-of-care, admission, routine, recertification, and discharge assessments using OASIS
• Deliver safe medication administration including intradermal, intramuscular, subcutaneous, and intravenous infusions
• Provide skilled wound care and V.A.C. management to promote healing and prevent complications
• Educate patients and caregivers on disease management, catheter care, G-tube and colostomy/ileostomy care
• Document accurately and timely in EPIC (remote) and Rover mobile app to meet productivity standards
• Coordinate care with the interdisciplinary team and arrange resources to support patient goals
• Apply infection control precautions and ensure safe home environments
• Manage patient visits efficiently (may see up to 5–6 patients/day) while prioritizing quality and patient-centered care

Qualifications

• Active RN license for Oregon (pending licenses accepted — confirm with hiring manager)
• Minimum 1 year of skilled Home Health nursing experience required (first-timers accepted if they meet core skills)
• BCLS (BLS) certification required
• OASIS documentation experience required — please call out if you do not have OASIS experience
• Familiarity with EPIC and Rover (phone app) strongly preferred
• Comfort with IV infusions, wound care, PCA pump management, PICC line care, and G-tube / ostomy care
• Own reliable transportation (occasional daily driving radius up to ~60 miles)
• Facility requires primary series COVID vaccination plus one booster

Education

• Current Registered Nurse (RN) licensure for Oregon (pending accepted)
• BCLS (BLS) certification

Knowledge & Skills

• OASIS documentation and start-of-care workflows
• EPIC remote charting and Rover mobile documentation
• Medication administration and assessment, including IV infusions
• Wound care, V.A.C. management, and integumentary assessment
• Colostomy/ileostomy and G-tube site care
• PICC line care and catheter education
• Pain assessment and use of pain scales (verbal and nonverbal)
• Infection control precautions and home safety assessment
