Roles & Responsibilities

• Implement treatment plans under the supervision of the Physical Therapist to help residents progress
• Provide therapeutic interventions to improve strength, mobility, balance, and daily function
• Document patient progress according to facility and state guidelines with accuracy
• Assist patients with therapeutic exercises and mobility training to boost independence
• Collaborate closely with the PT, nursing staff, and interdisciplinary team to deliver coordinated care
• Educate patients and caregivers on safe mobility techniques and home exercise programs

Qualifications

• Active California PTA license required
• SNF experience preferred
• Strong communication skills and solid clinical judgment
• Good organizational skills and reliable documentation practices
• Compassionate, dependable, and patient-centered approach

Knowledge & Skills

• Foundational knowledge of physical therapy principles and therapeutic interventions
• Patient care skills for mobility training and exercise instruction
• Clinical documentation and compliance with facility and state guidelines
• Interdisciplinary collaboration and effective communication with care teams
• Safety-focused patient education and fall prevention strategies
