Roles & Responsibilities
- Conduct thorough evaluations and develop individualized, goal-driven treatment plans
- Provide therapeutic interventions focused on ADLs, safety, mobility, and functional improvement
- Complete timely and accurate documentation according to facility and regulatory guidelines
- Guide residents through meaningful therapeutic activities and adaptive strategies
- Collaborate with COTAs, nursing staff, and the interdisciplinary team to optimize outcomes
- Educate residents and caregivers to support long-term independence and success
Qualifications
- Active California OT License (required) - must be current and in good standing
- Prior SNF experience preferred and valued
- Strong communication skills to coordinate care with residents, families, and the team
- Sound clinical judgment and organizational skills to manage caseloads effectively
- Dependable, compassionate, and patient-centered approach to rehabilitation
Knowledge & Skills
- Skilled in ADLs assessment and interventions
- Focused on resident safety and fall risk mitigation
- Experience improving mobility and functional outcomes
- Excellent clinical documentation and regulatory compliance
- Collaborative teamwork with COTAs, nursing, and interdisciplinary staff
- Strong communication and organizational abilities
