Roles & Responsibilities
- Coordinate and manage inpatient case management activities to ensure safe, timely discharges
- Perform utilization review and apply InterQual and Milliman criteria to support appropriate care decisions
- Document patient care, care plans and discharge plans accurately in Epic
- Collaborate with physicians, social services, transportation and clinical teams to remove barriers to discharge
- Advocate for patients and families to secure appropriate post-acute placements including home health and SNF
- Ensure regulatory and payer compliance, including CMS, DRG, CPT and ICD-10 requirements
- Provide education and care coordination to support continuity across settings
Qualifications
- Active California RN license required
- Minimum 3 years of inpatient case management experience
- Epic experience required (charting in Epic)
- Knowledge of HMO and Medicare in inpatient, home health and SNF settings
- BLS preferred
- Experience with discharge planning, utilization review and care coordination
Education
- Registered Nurse (RN) — nursing diploma, ADN or BSN (RN licensure in CA required)
Knowledge & Skills
- Strong working knowledge of InterQual and Milliman guidelines
- Familiarity with CMS rules, DRG, CPT coding and ICD-10
- Understanding of HIPAA, NCQA standards, OSHA and The Joint Commission core measures
- Excellent clinical documentation skills and attention to regulatory detail
- Able to collaborate across interdisciplinary teams and coordinate complex discharges
