Roles & Responsibilities

• Implement individualized treatment plans developed by the physical therapist to help residents regain mobility and independence
• Provide therapeutic exercises, gait training, balance and transfer training with safety and compassion
• Document patient progress accurately and timely in the electronic medical record
• Communicate regularly with interdisciplinary team members and family to coordinate care
• Monitor and modify interventions based on resident response and safety considerations
• Educate residents and caregivers on home exercise programs and safe mobility techniques

Qualifications

• Valid Physical Therapist Assistant (PTA) license for Pennsylvania required
• Current CPR/BLS certification required
• 1+ year of experience in skilled nursing, long term care, or rehab settings preferred
• Ability to pass background check and required health screenings or immunizations
• Strong communication skills and a collaborative mindset

Education

• Associate Degree in Physical Therapist Assistant or accredited equivalent

Knowledge & Skills

• Proficiency with therapeutic exercise techniques, gait training, transfers and ADL assistance
• Knowledge of safety and fall prevention practices
• Comfortable documenting in EMR systems and completing clinical progress notes
• Ability to adapt treatment plans to individual resident needs
• Time management skills to balance caseload and documentation
