Roles & Responsibilities
- Evaluate patients' mobility, strength, balance, and functional needs and document findings clearly
- Develop and implement individualized treatment plans with measurable goals
- Deliver evidence-based therapeutic interventions including therapeutic exercise, gait training, and manual therapy
- Monitor patient progress and adjust treatment plans to maximize outcomes
- Collaborate with the interdisciplinary care team to support discharge planning and continuity of care
- Educate patients and families on home exercise programs and injury prevention strategies
- Maintain accurate, timely documentation in the facility's medical record system
- Promote a safe environment and adhere to facility policies and professional standards
Qualifications
- Current, active New Mexico Physical Therapist license (required)
- Doctor of Physical Therapy (DPT) preferred; Bachelor's or Master's in Physical Therapy accepted
- Current BLS/CPR certification
- Previous experience in an outpatient, skilled nursing, or acute care setting preferred
- Strong communication skills and ability to work collaboratively with a multidisciplinary team
- Ability to pass background checks and meet agency credentialing requirements
Education
- Doctor of Physical Therapy (DPT) preferred; Master's or Bachelor's in Physical Therapy acceptable
Knowledge & Skills
- Proficiency with therapeutic exercise, manual therapy techniques, and gait training
- Familiarity with common modalities and assistive devices
- Comfortable with patient assessment, care planning, and measurable outcome documentation
- Experience using electronic medical records for documentation
- Strong clinical reasoning, organization, and time-management skills
- Compassionate patient education and effective family communication
