Roles & Responsibilities

• Provide compassionate direct patient care, focusing on safety and dignity
• Administer medications and document treatments accurately
• Monitor resident condition and communicate changes to the care team
• Support activities of daily living and create a comforting environment
• Follow infection control protocols and facility policies to keep everyone safe
• Collaborate with nurses and providers to develop practical care plans
• Use electronic charting to maintain clear, up-to-date clinical records

Qualifications

• Current LPN license in New Mexico or eligibility for New Mexico licensure
• Graduation from an accredited LPN program
• Current BLS/CPR certification
• Prior experience in long term care or similar clinical setting preferred
• Ability to pass background checks and meet facility onboarding requirements
• Strong communication skills and a team-focused attitude

Education

• Graduate of an accredited LPN program
• High School Diploma or GED

Knowledge & Skills

• Skilled in medication administration and basic clinical assessments
• Experience with ADL support and resident-centered care
• Familiarity with infection prevention and safety best practices
• Comfortable with electronic health record charting
• Good time management and prioritization during busy shifts
• Professional bedside manner and clear communication with families
