Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to rehab patients and support their recovery goals
• Perform skilled assessments, monitor changes in condition, and communicate findings to the interdisciplinary team
• Administer medications safely and according to policy, documenting all treatments accurately
• Collaborate with therapists and physicians to implement individualized care plans that advance patient independence
• Educate patients and families on care plans, discharge instructions, and ongoing self-care strategies
• Maintain crisp, timely documentation in the electronic health record and contribute to quality improvement efforts

Qualifications

• Active RN or LPN license in New Mexico (RN preferred for this posting)
• Current BLS (Basic Life Support) certification required
• ACLS preferred but not required
• Minimum 1 year of clinical nursing experience; experience in rehabilitation or skilled nursing is a strong plus
• Up-to-date immunizations and ability to pass facility background checks
• Excellent communication skills and a collaborative attitude

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
• LPN candidates must hold an accredited practical nursing diploma/certificate

Knowledge & Skills

• Patient assessment and care planning in a rehab setting
• Medication administration and IV/PO management
• Wound care and mobility/safety interventions
• Familiarity with electronic health records; experience with Epic or similar systems preferred
• Strong teamwork, time management, and patient education skills
• Ability to prioritize, multitask, and adapt in a fast-paced clinical environment
