Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to rehab patients and support their recovery goals
- Perform skilled assessments, monitor changes in condition, and communicate findings to the interdisciplinary team
- Administer medications safely and according to policy, documenting all treatments accurately
- Collaborate with therapists and physicians to implement individualized care plans that advance patient independence
- Educate patients and families on care plans, discharge instructions, and ongoing self-care strategies
- Maintain crisp, timely documentation in the electronic health record and contribute to quality improvement efforts
Qualifications
- Active RN or LPN license in New Mexico (RN preferred for this posting)
- Current BLS (Basic Life Support) certification required
- ACLS preferred but not required
- Minimum 1 year of clinical nursing experience; experience in rehabilitation or skilled nursing is a strong plus
- Up-to-date immunizations and ability to pass facility background checks
- Excellent communication skills and a collaborative attitude
Education
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
- LPN candidates must hold an accredited practical nursing diploma/certificate
Knowledge & Skills
- Patient assessment and care planning in a rehab setting
- Medication administration and IV/PO management
- Wound care and mobility/safety interventions
- Familiarity with electronic health records; experience with Epic or similar systems preferred
- Strong teamwork, time management, and patient education skills
- Ability to prioritize, multitask, and adapt in a fast-paced clinical environment
