Roles & Responsibilities
- Provide compassionate, skilled nursing care to residents in a skilled nursing setting
- Administer medications and treatments accurately and safely
- Assess resident condition and document changes in the medical record
- Collaborate with the interdisciplinary team to create and follow individualized care plans
- Support residents with activities of daily living while promoting independence
- Monitor vital signs and respond promptly to changes in condition
- Communicate clearly and kindly with residents, families, and team members
Qualifications
- Current LPN license for Maryland in good standing
- Basic Life Support (BLS) certification required
- Previous experience in long-term care or skilled nursing preferred
- Current immunizations and ability to pass background/health screenings
- Strong communication skills and a team-oriented attitude
Education
- Graduate of an accredited LPN program; current Maryland LPN licensure required
Knowledge & Skills
- Medication administration and documentation
- Wound care basics and infection control practices
- Proficiency with electronic medical records and charting
- Assessment skills for changes in cognitive and physical status
- Time management and prioritization in a busy clinical environment
- Excellent interpersonal skills for interacting with residents and families
