Roles & Responsibilities
- Assist the Occupational Therapist in implementing individualized treatment plans to help residents regain independence
- Deliver therapeutic exercises and functional training that make daily life easier for patients
- Maintain accurate and timely documentation to support quality care and continuity
- Participate in interdisciplinary care team coordination to ensure cohesive patient-centered care
- Provide patient and family education to empower recovery and improve outcomes
Qualifications
- Active California COTA license required
- Prior Skilled Nursing Facility (SNF) experience preferred
- Strong interpersonal and clinical communication skills
- Ability to work collaboratively in a rehab setting with therapists and care teams
- Reliable documentation and time-management skills
Knowledge & Skills
- Foundational occupational therapy and rehabilitation techniques
- Patient-centered care and functional training methods
- Therapeutic exercise delivery and adaptive equipment knowledge
- Clear clinical documentation and charting practices
- Effective communication with patients, families, and interdisciplinary teams
- Ability to prioritize tasks in a busy SNF environment
