Roles & Responsibilities
- Prepare and dispense medication orders accurately and safely for inpatient care
- Maintain medication stock in patient care areas to ensure timely therapy
- Compound sterile products, including IV admixtures, chemotherapy, and parenteral nutrition
- Interpret medication orders and maintain computerized patient medication profiles
- Monitor drug therapy for contraindications, interactions, allergies and appropriate dosing
- Perform pharmacokinetic dosing according to hospital protocol
- Review culture & sensitivity data and recommend appropriate antibiotic therapy
- Provide clinical consultation and cost-effective therapeutic alternatives to prescribers
- Document clinical activities and interventions clearly and promptly
- Participate in quality improvement, medication reviews, and be a member of the Code Response Team
Qualifications
- Active Colorado Pharmacist (PHA) license in good standing — submissions must include license
- Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree (per job posting)
- BLS (American Heart Association) required
- ACLS (American Heart Association) required
- Flu vaccine REQUIRED (no exemptions) — seasonal requirement Oct 1 through Apr 1; proof required with submission
- Willingness to work nights (7-on/7-off Night schedule)
- Experience compounding sterile products, chemotherapy handling and parenteral nutrition preferred
- Strong clinical assessment, documentation and medication safety skills
Education
- BS Pharmacy from an ACPE-accredited School of Pharmacy

- Medication therapy management and clinical consultation
- Formulary management and therapeutic substitution
- Rational drug therapy selection and cost-conscious recommendations
- IV admixture preparation, chemotherapy handling, parenteral nutrition
- Pharmacokinetic dosing and therapeutic drug monitoring
- Antibiotic stewardship and culture & sensitivity interpretation
- Adverse drug reaction detection and reporting
- Controlled substance handling and regulatory compliance
- Accurate chart review and computerized medication profile management
