Roles & Responsibilities

• Assess communication and swallowing function for residents in a skilled nursing setting
• Develop individualized, evidence-based treatment plans to improve patient outcomes
• Implement therapy interventions and adapt approaches to each resident's needs
• Document evaluations, plans, and progress clearly and timely in the patient record
• Collaborate with nurses, therapists, and interdisciplinary team members to coordinate care
• Educate residents and families about therapy goals, carryover activities, and safety
• Meet productivity expectations and contribute positively to the therapy team's goals

Qualifications

• Primary skill: SLP (Speech-Language Pathology) — experience working as an SLP
• 1-3 years of clinical experience (as listed: 1-3 years)
• Experience or comfort working in a skilled nursing facility (SNF) practice setting
• Willingness to follow Continuum Rehabilitation Group policies & procedures
• Ability to meet a therapist productivity requirement (90%) and be present on-site to receive guaranteed hours
• Professional appearance (dress code: black scrubs) and reliable attendance

Knowledge & Skills

• Strong assessment and treatment planning skills for adult populations
• Experience with documentation and productivity systems in SNF settings
• Effective communication and teamwork with interdisciplinary staff
• Ability to provide patient- and family-centered education
• Time management and organizational skills to manage caseload and documentation
