Roles & Responsibilities

• Conduct thorough clinical screening, assessment, diagnosis, and treatment planning for clients from birth through elder
• Provide brief therapy and continuing care to support recovery and resilience
• Lead and facilitate patient care conferences and discharge planning, including ALF/transitional placement coordination
• Coordinate care with hospital teams and community partners to secure the right services at the right time
• Advocate for clients and assist with complex matters such as Power of Attorney when relevant
• Maintain accurate, timely, and compliant documentation to meet state, federal, accreditation, and billing standards
• Support clients with chemical dependency and mental health needs through appropriate referrals and interventions
• Participate in multidisciplinary team meetings and help develop and monitor individualized treatment plans
• Contribute to staff training, quality management initiatives, and integration of best practices into care delivery
• Complete required provider enrollment and program documentation in a timely, professional manner

Qualifications

• Master’s degree in Social Work, Psychology, Counseling, or a related field from an accredited institution
• Licensed (or in-process) as an LCSW, LPC, or LMFT
• Valid State of Alaska driver’s license or able to obtain within 6 months of hire
• Must be at least 21 years of age
• Clinical Competency assessment required within 3 months of hire and every three years thereafter
• Experience with discharge planning, care coordination, case management, and client advocacy
• Comfort working within tribal health settings and compliance with Indian Child Protection Act (ICPA) requirements

Education

• Master’s Degree in Social Work, Psychology, Counseling, or related discipline (accredited institution)

• Care coordination and multidisciplinary team collaboration
• Documentation practices that meet accreditation and hospital billing requirements

• Client advocacy, skill-building, and complex case navigation
