Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate patient care and services
Develop and implement patient care plans
Collaborate with healthcare teams
Utilize Midas and Meditech systems
Plan for complex discharges

Qualifications

1 year of experience in Case Management
RN license
Midas and Meditech experience required
Complex discharge planning experience preferred
