Roles & Responsibilities

• Lead and coordinate the Long-Term Care QAPI program to drive meaningful quality improvements
• Plan, organize, and direct quality assurance activities and accreditation readiness efforts
• Facilitate regulatory surveys and support continuous compliance with CMS and other bodies
• Analyze clinical outcome data and identify trends to prioritize performance improvement projects
• Design and maintain the formal QAPI plan and track action items from quality committees
• Conduct and document Root Cause Analyses (RCAs) and support investigations of incidents and complaints
• Perform chart audits, rounding, and other monitoring activities to ensure standards of care
• Collaborate with Administration, Providers, Leaders and Staff to coach, educate, and integrate QM into daily practice
• Review and revise policies, procedures, and survey reports to maintain accreditation readiness
• Organize and facilitate quality committee meetings and maintain clear meeting minutes and follow-up

Qualifications

• Current full, unrestricted Registered Nurse (RN) license in Alaska (active or applied for prior to start for direct hire)
• CPHQ or QAPI Certified Professional (QCP) or other LTC quality certification obtained within 3 years (required)
• Minimum 2 years' experience monitoring quality and regulatory requirements, planning and coordinating process changes
• Demonstrated leadership experience facilitating cross-functional teams and quality initiatives
• Basic Life Support (BLS) certification (American Heart Association)
• Experience with chart audits, Datix/event management, and incident investigation preferred
• Preferred experience working in Long-Term Care (LTC)

Education

• Active Registered Nurse (RN) license — State of Alaska (required)
• CPHQ or QCP or equivalent LTC quality certification (required within 3 years)

Knowledge & Skills

• Strong knowledge of accreditation and regulatory requirements across long-term care and healthcare settings
• Solid understanding of quality improvement methodologies and performance measurement
• Skilled in data analytics: collection, analysis, and presentation of clinical and performance data
• Proficient with Microsoft Office (Excel, Word, PowerPoint) for reporting and presentations
• Excellent verbal and written communication, presentation, and facilitation skills
• Ability to lead group processes, mentor staff, and integrate quality into daily operations
• Familiarity with infection control practices and clinical competency assessment
