Roles & Responsibilities
- Provide high-quality patient care and case management services
- Collaborate with healthcare teams to develop and implement patient care plans
- Utilize strong communication and interpersonal skills to interact with patients, families, and healthcare professionals
Qualifications
- Current RN license in the state of Texas
- 1 year of experience in case management or a related field
- Strong clinical knowledge and case management skills
Education
- Associate's or Bachelor's degree in Nursing
Knowledge & Skills
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Knowledge of case management principles and practices
