Roles & Responsibilities

• Evaluate patients' functional status and develop individualized, goal-oriented treatment plans
• Provide skilled therapeutic interventions to improve mobility, strength, balance, and independence
• Document patient progress clearly and timely to meet facility and regulatory standards
• Collaborate with nurses, OT, SLP, and care teams to coordinate comprehensive care
• Meet productivity expectations and contribute positively to the therapy team (90% productivity requirement)
• Follow CRG policies and procedures, including dress code and on-site presence requirements
• Communicate with families and caregivers to support safe discharge planning and carryover

Qualifications

• Licensed Physical Therapist (PT) in New Mexico or eligible to obtain state licensure
• DPT preferred; PT (entry-level) candidates with relevant SNF experience considered
• 1–3 years of experience working in skilled nursing or post-acute settings preferred
• Current BLS/CPR certification (or ability to obtain prior to start)
• Reliable, professional, and able to maintain required productivity standards
• Willingness to follow facility dress code (black scrubs) and be present onsite to receive guaranteed hours
• Able to pass background checks and meet credentialing requirements

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent PT qualification

Knowledge & Skills

• Skilled in PT assessment, therapeutic exercise, gait training, and mobility strategies
• Documentation skills for SNF charting and regulatory compliance
• Knowledge of post-acute care workflows and interdisciplinary collaboration
• Strong communication skills with patients, families, and staff
• Time management and caseload prioritization to meet productivity targets
• Comfortable working within CRG policies and facility-specific procedures
