Roles & Responsibilities

• Deliver skilled, patient-centered physical therapy services to residents in a skilled nursing facility
• Assess functional mobility and develop individualized treatment plans to meet rehab goals
• Provide hands-on therapeutic interventions and progressive exercise programs
• Document patient progress accurately and timely in the facility charting system
• Collaborate with nursing, OT, SLP, and other interdisciplinary team members to optimize patient outcomes
• Educate patients and families on home programs, safety, and fall prevention
• Flex to cover different units or caseloads as needed to support resident care

Qualifications

• Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT); new grads are welcome
• Eligible for or willing to obtain Tennessee licensure in a timely manner
• Comfortable working in a Skilled Nursing Facility (SNF) setting; SNF experience is a plus
• Ability to float within the facility when requested
• Excellent communication skills and a collaborative mindset

Education

• Doctor of Physical Therapy (DPT) preferred; new graduates considered

Knowledge & Skills

• Strong knowledge of geriatric rehabilitation and functional mobility interventions
• Skilled in assessment, treatment planning, and progress documentation
• Familiarity with common SNF patient populations (post-acute, medically complex, fall risk)
• Ability to work effectively within an interdisciplinary team
• Time management and prioritization in a busy clinical setting
