Roles & Responsibilities

Deliver impactful physical therapy to patients in rural and mid-sized communities across four counties
Conduct OASIS assessments and start-of-care evaluations with precision and care
Manage a rewarding caseload of 4-6 patient visits daily using innovative tablet technology
Specialize in common cases like elderly falls and fractures, making real differences in lives
Track progress and time efficiently to ensure top-quality home health services

Qualifications

Active Physical Therapist (PT) license in Indiana
Experience with OASIS assessments and start-of-care processes
Comfortable with tablet-based documentation and point systems
Ready to embrace business casual or Hunter green scrubs with white or black top

Education

Physical Therapy degree from an accredited program
State licensure as a Physical Therapist

Knowledge & Skills

Expertise in home health physical therapy
Proficient in OASIS assessments and start-of-care
Skilled in treating orthopedic cases like falls and fractures
Tech-savvy with tablet tracking for visits and time
